What we offer in Form

Questionnaires in Medibrix can be easily customized to your clinic’s needs. You have access to a variety of question types and can add smart logic that adapts the questions along the way. Based on the customer’s answers, only relevant follow-up questions are displayed, which provides a more precise and efficient information gathering.
Each form can automatically generate a structured summary based on the client’s responses. The layout can be customized with the desired structure and number of paragraphs to suit the clinic’s routines. The summary can be used as a draft journal note and edited as needed. All information is stored securely and in compliance with regulations in Medibrix.
Forms can be sent to the customer for completion in advance, or completed on behalf of the customer directly in Medibrix. The response can be linked to a specific appointment via Medibrix Calendar, or automatically added to the Medibrix Worklist. This provides flexible workflows and full overview, regardless of how the form is answered.
